Prior to your first visit, we ask that you please fill out the forms located in the New Patient Center and bring them with you to your initial consultation.
This paperwork provides us with your health history and information on your present condition
This will help us accelerate the intake process and eliminate the possibility of a wait time.
We also ask that you please call our office in advance with any available insurance information.
Prior authorization may be required before your visit.
First, you will have a consultation with Dr. David Ackerman to discuss your health-related problems and concerns.
This initial visit is designed for Dr. Ackerman to learn more about you, your condition, and to determine if chiropractic care is right for you.
Once he has established that he can help you, he will let you know of any fees associated with his examination before he continues.

After your consultation, Dr. Ackerman will perform his evaluation including a comprehensive health history, dietary assessment, postural assessment, range of motion evaluation, computerized foot scanning, pH balance testing and X-rays (only if necessary).
Once the information is collected and all necessary examinations are performed, Dr. Ackerman will give you a detailed report of all findings and answer any questions that you may have.
He will also discuss some of the many treatment options available for your specific condition.
After your initial consultation, if X-rays are not required, you will be eligible to receive your first treatment.
All of our treatments start with the least invasive techniques and are based on your comfort levels and condition.
Active Care is a huge part of our office hence the name "Active Chiropractic Care."
At some point you will be required to play a part in your care. You can start doing this by making a conscious effort to enforce the new lifestyle which we are working together to designed for you.
This may include recommendations such as dietary modifications, supplement prescriptions, stretches, exercises, stabilizing foot orthotics, and other lifestyle modifications.
